He made his first theatrical film appearance in the 2013 Tamil-language horror-thriller film Pizza II: Villa, starring Ashok Selvan and Sanchita Shetty. However, his role went uncredited. The film also marked K. Manikandan’s debut as a writer.
In 2015, he made his first credited film appearance by playing the role of Mani in the Tamil-language romantic comedy film India Pakistan.
Alongside Suriya and Lijomol Jose, K. Manikandan co-starred as Rajakannu (Sengeni’s husband who dies in police custody) in the 2021 Tamil-language legal drama film Jai Bhim. The film is based on a true incident in 1993 and its title is a reference to the slogan used by followers of Indian social reformer and political leader B. R. Ambedkar.
For his performance in Jai Bhim, K. Manikandan was nominated for a Filmfare Award South for “Best Actor – Tamil” and a South Indian International Movie Award for “Best Supporting Actor – Tamil”.
